Prescription Request
This service is for existing patients of Dr Chan. Only medications relevant to your rheumatological condition will be prescribed. Please see your GP for all other medications. Rheumatological medications will only be prescribed if you have had a normal blood test result within the last 3 months. Each prescription issued will only cover a maximum of 3 months. If you require prescriptions before your next appointment with Dr Chan, please either seek one from your GP or contact us here for this service.
There is a $40 charge for this service.
We cannot email the prescription to you. The prescription will be sent directly to your pharmacy. Your details and medications must match our records. Please allow up to 7 days for us to action this request.
